Delivery

Installation
I {{first_name}} {{last_name}} agree that the equipment has been fully installed in my designated working area and is correct as per the manufacturers instruction and in working manner.
Instruction
I {{first_name}} {{last_name}} agree that I have been properly instructed on how to use all equipment that has been delivered and installed. I agree to use said equipment as instructed and not use it outside my working practices or outside the intended use of said equipment.

I understand and agree that the statements and instruction herein are true, were properly and fully conducted. I hereby agree that any change to the condition of the equipment does not give any liability to Homedeskr and / or the installer and that any such change will be reported to my line manager or responsible person(s) at my company {{shipping_company_name}}. I also agree that any injury or damage to property or equipment by misuse or improper conduct of said equipment does not give any liability to Homedeskr and / or the installer.
